Each bundle includes any mix of DOT or Non-DOT tests (rapid or lab-based), and employers may add more tests as needed at their locked-in rate.

• Employers are billed monthly on the 1st of each month.
   
• Unused tests do not roll over, but you can upgrade or downgrade at any time.
   
• Additional tests beyond your plan are billed at your plan’s locked-in rate.
   
• Bundle pricing includes standard reporting and administrative handling.
   
• Mobile, after-hours, or confirmation fees are billed separately as needed.

• Billing Options:
   
  • Pay-per-test (invoiced weekly or monthly depending on approval method)
     
  • Monthly Bundle Subscription (billed automatically)
     
• Payment Terms: NET 15 for pay-per-test clients; due upon receipt for bundles.
   
• Late Payments: $150 late fee after 8 days past due.and every 15 days is a additional $150 until balance is paid in full
   
• Accepted Payments: Square, ACH, Credit Card, or Business Check.
   
• Cancellation Policy: Monthly bundles may be cancelled with 30 days’ notice.
